Yes. Modifications will be requested through the Academic Monitoring Portal for Doctoral Students in the ‘Solicitudes’ (Applications) tab. They will be sent to the academic committee of the programme for authorisation.
In relation to this issue, Article 13.4 of Royal Decree 99/2011 establishes:
The thesis may be developed and, where appropriate, defended, in the common languages for scientific communication in its field of knowledge. In addition, University of Granada regulations require that a comprehensive summary in Spanish be included if the thesis is written in another language.
For doctoral students admitted in accordance with Royal Decree 99/2011, a deadline of 3 years full-time or 5 years part-time is established, with the possibility of an authorised extension. In any case, there is a minimum period of two years from the time of the student's enrolment until the thesis can be defended.

By providing documentation accrediting the publication (if it is published, with a copy of the published article; if it is accepted/pending publication, with a copy of the article and proof of acceptance), as well as documentary justification that the publication fits the categories indicated in the field of knowledge (that is, a copy of the ISI-JCR indicating the impact factor and quartile justifying its appearance in other repertories and its classification in them, etc.).
Doctoral students can obtain it in the Academic Monitoring Portal for Doctoral Students in the ’Documento de Actividades (DAD)’ (Doctoral Student’s Activity Document) tab.
The requirements for this are outlined in Article 15 of Royal Decree 99/11.

The naming of the degree is established under Royal Decree 99/2011 and will be “Doctor por la Universidad de Granada” (Doctor of the University of Granada) and will indicate the doctoral programme completed.

Yes. Both supervise and are therefore equally responsible. A maximum of 3 thesis supervisors are allowed. See the rules of each Doctoral School.
Yes. No additional permission, procedure or format is required to that effect.
However, a comprehensive summary and conclusions in Spanish should be included in this case.
Yes. A translation is only necessary if they are received in a language other than Spanish or English.
