Application form for refund of fees

• Fill in the required details. You must complete the form with YOUR details, not those of the bank account holder. Make sure you indicate the reason for the refund and the bank details in IBAN format.

• IMPORTANT: The IBAN code must belong to a bank account located in Spain. Therefore, it must begin with “ES”.

• Convert the file to PDF format.
